Before you arrive: 

​

• If you are feeling ill with any of the following symptoms, you will need to be rescheduled. Thank you for your understanding. 

  • Fever above 100.4F​

  • Cough

  • Shortness of breath

  • Loss of taste or smell

  • Chest pain ​

• All personnel routinely washes hands and thoroughly disinfect all high-touched surfaces. ​

• Exam lanes are disinfected before and after each patient appointment.

When you arrive:

​

• Face Mask is required. If you do not have one, one will be provided. 

• Hand sanitizers available to sanitize upon entry. 

• Temperature checks will be conducted. If you have a temperature at or above 100.4F, you will be asked to reschedule and reach out to your primary care provider. 

• Please minimize having any additional family members with you during your appointment, unless absolutely necessary.
